First of all, the patient should be offered surgery and there should be an important justification for the surgery. Initial treatment is medical treatment, findings in which medical treatment is inadequate, can no longer lead a normal life with medication, have frequent on and off fluctuations, or medical treatment is no longer effective due to intense drug side effects are candidates for surgical treatment. At the same time, if there is very severe tremor at the beginning and this does not improve with any kind of medical treatment, Deep Brain Stimulation can be used to stop it.

Parkinson's Plus or patients that have other problems in addition to Parkinson's are not suitable for such a surgery, because they do not respond to medical treatment,and have problems that are different from Parkinson's due to some features and that make it progress rapidly. Apart from this, even if the patient is a candidate for surgery, if a systemic disorder prevents the application of Deep Brain Stimulation treatment or if there are other internal and systemic diseases that cannot take short-term anesthesia during that procedure, it is not correct to apply Deep Brain Stimulation therapy in these patients, other treatment methods may need to be considered.
