DBS:

Technological Touch on Movement Disorders

With this method called "neuromodulation", it is possible to turn back the clock of the disease in Parkinson's, Essential Tremor and Dystonia diseases and say "hello" to life again.

Don't Be Detached From Life

Reconnect to Life with Talk to Talk Surgery

The patient is awake during the entire phase of the surgery where the DBS electrodes are placed. Our aim here is to measure the patient's response to the given electric current. During surgery, our patients sometimes talk about their grandchildren, they chat about football, and they sing and tell jokes.

The Change Begins

Reach Your New Life in Just 4 Hours of Surgery

DBS implantation surgeries are  completed in 3-4 hours on average. After 1 to 2 days of hospitalization, the  patient is discharged and we begin outpatient examinations and DBS  programming. After about 3 weeks, our patients return to their homes.

From Where You Left off

Return to Life After DBS Treatment

There  is no change in the daily life of patients who have a DBS inserted. Patients  can engage in all kinds of sportive activities that are not too hard and do  not cause head trauma, they can play tennis, ride a bicycle and swim.

Assoc. Prof. Dr.
Ali Zırh

More than 30 years of clinical and surgical experience in the field of functional neurosurgery, which is his specialty to date, including his work at John Hopkins University in the USA; More than 1900 surgical interventions for Parkinson's and other movement disorders and more than 1400 DBS surgery applications, which he has successfully performed, made Dr. Ali Zırh the first name that comes to mind in movement disorder surgery in our country, and he enabled the center where he performs these surgeries to become the International Training and Reference Center in the international arena.

Born in Ankara in 1960, Assoc. Prof. Dr. Ali Zırh completed his primary and secondary education in Ankara and graduated from Hacettepe University Faculty of Medicine as a Doctor of Medicine in 1983. Since 1988, he has participated in post-graduate  and clinical studies in the field of neurosurgery in different parts of the world, becoming a neurosurgery specialist in 1994 and completing his master's degree at Marmara University.

Assoc. Prof. Dr. Ali Zırh returned from the United States in 1997 and started to work at the VKV American Hospital, performing functional neurosurgery operations and DBS implantations with the microelectrode recording and stimulation technique at the single cell level which  located in only a few centers in the world. While participating in congresses, symposiums, and meetings to share his knowledge with his colleagues, he also closely followed developments in his field of expertise.

Have you met the DBS Technology?

Deep Brain Stimulators are devices that enable us to deliver electrical current to any point in the human brain, thus stimulating or suppressing the electrical activity in the brain cells in the area where the electrical current is applied. We can successfully treat Parkinson's disease as well as movement disorders such as essential tremor and dystonia using this treatment method known as "neuromodulation." DBS is a device placed inside the human brain, consisting of an electrode with four polarity-variable platinum-iridium poles, an extension that connects this electrode to the main battery device, and the body of the DBS. The electrode part of the DBS is placed inside the human brain. This electrode is integrated into the battery placed under the skin on the upper part of the rib cage by means of an extension. This battery device, which can be programmed with an external computer, enables the four poles at the end of the electrode to be changed as positive, negative, or neutral, and to produce alternative combinations. In addition, the intensity of the electric current to be given to the brain, the number of waves given per second, and the wave width are adjustable.

How are the Positions of the Target Cells Found?

In these surgeries, we find an anatomical formation with a diameter of 2-3 millimeters in the brain and intervene there. The safest method, which ensures that the results are very pleasing and the low risks, that is, "ensure the right intervention in the right place," is the "Microelectrode Recording and Stimulation Technique." Thanks to this technique, we can listen to the cells responsible for the disease in the brain one by one and determine their correct locations. This technology brings us to our goal, and thanks to this technique, we can draw the physiological map of the brain. We place DBS electrodes on the diseased area, which we detected with an error margin of less than 80 microns.

Why Is The Patient Awake During Surgery?

During most of the operation, the patient is awake and talking to us and we’re chatting with each other. Sometimes they talk about their grandchildren, sometimes they chat about football and sometimes they sing a song. The purpose of keeping the patient awake during surgery is to locate the cells responsible for the disease and the anatomical formations around it. For this reason, we perform the surgery while the patient is awake and talking. In this way, it is easier to reach the problematic area by measuring the patient's reactions.

What is the Technological Life-time of DBS?

Until recently, we used non-rechargeable batteries, and the lifespan of DBS in Parkinson's patients was between 4-6 years. This period may be even shorter in dystonia patients who require higher electrical current. Thanks to the developing technology in recent years, it is possible to apply batteries that can be charged externally and have a life-time of up to 20-25 years; also we have started to use devices with technology that can adjust the direction and intensity of the electric current we inject into the brain as we want. Thanks to these developments, our success rates in treatment have increased. On the other hand, there has been no need for battery replacement for a long time.

The Importance and Function of the DBS

We can define DBS surgeries as "turning back the clock of the disease". We can return a ten years Parkinson's patient back to the early years of her/his disease. We do not eliminate the disease, but significantly improve the motor manifestations of it. Thanks to this treatment, people who cannot live independently and who are disconnected from social life have the chance to live independently again, and a significant number of them have the chance to practice their profession again.

What Do
Our Patients Say?

The diagnose of Parkinson’s disease stole 10 years of my life. The doctors said that the tremor I felt in my hand was psychological and started psychological treatment.I said I couldn't smell; they did my nose surgery; I said I couldn't hold my hand and they sent me to physical therapy.I finally found out that I have Parkinson's. My quality of life began to decline rapidly. My brother reached out to Ali ZIRH online. While I couldn't even drink my own water while going into surgery, I am currently making tiles and painting ceramics. Thank you very much to Dr.Ali.

I battled with Parkinson's for 7 years. When the treatment with drugs was insufficient, I decided to meet with Dr. Ali Zırh. As a result of the examinations, my dear Doctor found it suitable for surgery. Before the surgery, "You will understand the difference when I start the procedure in the operating room." I had no idea that I would be so relieved when he said that. I got up right after the surgery. Now I can lead my own life.

With my wife, who has been suffering from Parkinson's disease for 16 years,as a result of the research of my daughter, who is a doctor, we met with Dr. Ali ZIRH. Our dear Doctor put a pacemaker in my wife. My wife was born again...It's been 6 years since the DBS was inserted, and the same performance continues. When the DBS is finished, it will be renewed with a small operation. The life of the new batteries is 25 years, and they can also be charged from the outside. What can I say? Technology is advancing rapidly, and my dear doctor, Ali ZIRH, follows technology closely. May God Bless doctors like Ali Zirh and keep them health

God bless Dr.Ali. He reconnected to life my daughter, Berfin, who is bedridden and embittered with dystonia, and us. Now my daughter goes to school and she meets all her needs herself. We are grateful for this every day.
